Try on another card or on other phone. Sometimes cards refuse to work correctly. Try to format it as Uli said. Also reboot your phone.
Check on your PC if files saved on card work fine. The easiest way is to fill it with pictures, reboot PC (to clear memory cache) and then watch pics from it. If they look fine card should be OK.
Are you using Walkman or other app?
