Watch out for Shazam! If you multi-pair your headphones with your Mac and you have the Shazam extension on your desktop toolbar then Shazam seems to automatically run and override your headphones, preventing you playing music on your other device, even when you're not actively using the Shazam extension. Probably because Shazam automatically listens in the background.
Quitting Shazam immediately fixed the problem.
