Have you tried gently pressing physical camera button while recording to focus?How about settings-apps-all-camera,clear data and restart the phone?
Yes, I did.
When pressing the button gently the camera "focuses" on the object (I have set the focus-point to "center", because it frequently chooses the wrong focal point), but even then the display is out-of-focus visually but the phone says "focussed"??? Refocussing several times ends up with a clear focus, but you try that with kids of 1 and 3, you miss a lot!!
Did a complete factory reset yesterday, because of the last update. It was recommended, but it makes no difference.
Somehow CameraMX does a better job, but also these pictures are of poor focus and blurred.
You might also want to try some apps which will add more focusing effect.I have never used them on my Xperia P,but you could give it a try.
I tried several Camera-Apps; CameraMX seemed to improve te pictures a bit, but still the camera is not very good.
Compared to my Samsung S2 or HTC Desire, this phone is bad.
Read elswhere to use ProCapture, will give that a try, but still I think my phone is defective (at lease the camera is) or the driver is no good. Seems more users are complaining about camera quality so I'm not the only one!

Try this.You might want to take it into service.
As I understand that app wil make the image out-of-focus in an area you select, which is of no use if the picture is already out of focus.
