I could not find anything in this forum regarding my problem:

I recently updated my Sony Xperia Z1 tot the new android lollypop!
But now when people call me and i don't answer my phone i don't get "missed call", it says call forwarded in my screen and in my call list it says "answered"(blue icon).
My settings are exactly the same as before the update (call forwarding for voicemail etc) and then it worked fine!

Also when i put my sim in my girlfriends phone (xperia z3 compact wit lollypop) and with exactly the same settings for voicemail i get "missed call" when i don't answer!

Please help me with this issue because it is very annoying when having your own company!

I just tried it in safe mode...

The message is the same when in normal mode:
(translated from dutch) "call unanswered and forwarded" and it shows a blue icon in call history.

It looks like it has something to do with the voicemail settings or so, but as i mentioned before when i put my sim in my girlfriend's xperia Z3 compact with exactly the same settings (automatically set by my provider?) it DOES work like normal...

Hey dude, I had the same problem. And it's because of the default setting set by your provider. I'll send you a pic what needs to be changed. Don't worry, it's really easy to fix.

Both my father and girlfriend own the Z3 Compact, and are having this issue. Now my GF unintentionally ignores (I hope) me due to this bug. Hopefully something can be done about this.
Also, similar to other people, this takes place in the Netherlands with KPN as provider.
